  1. Team Application & Entry Fee  
   
- On-line submission of Tournament Application.
- Payment must be received in order to be considered for the tournament field.
 
  2. Tournament Rules  
   
- Download and print rules from the Tournament Website.
- Make copy for coach and players to review.
 
  3. Team Roster  
   
- Approved State USYSA Roster (this must be your "official" state approved roster with seal/stamp), US Club roster or Candian Soccer Association (CSA) roster - (copy must be left at registration)
 
  4. NT Form  
   
- Must be filled out for all US CLub and CSA carded teams - download form from tournament web site under "forms" (copy must be left at registration)
 
  5. "Application to Travel" for USYSA Carded out of State Teams/CSA Carded Teams  
   
- Approved USYSA Form provided by your state association or CSA approved form (copy must be left at registration)
 
  6. Player Passes  
   
- Current/valid USYSA, US Club Soccer, or CSA Player Passes required for all players AND coaches.
- Approved Rosters and Player Passes must be from the same sanctioning organization (USYSA, US Club, or CSA)
 
  7. Tournament Roster  
   
- ALL teams must enter their team data into the "on-line NSR Region II Cup Tournament Roster" on the website.
- The link can be found under "Applications" or "Forms" on the tournament web page - listed as "Update/Add roster" (link available after accepted teams posted)

Note:  Only those players participating in the tournament should be listed - enter information by April 27, 2011
 
  8. Medical Release Forms

Present a Copy of the IYSA (Illinois Yourth Soccer Association) Medical Release for each player.  The IYSA Medical Release Form is available for download on the turnament web page under "forms".  (Copies must be left at registration - team representative should keep originals.)  Copies of insurance cards are not required.
 
  9. Guest Players for USYSA Carded Teams  
   
- Complete a guest player form approved by your state association.
 
10. Guest Players for US Club Carded Teams
   
- All players should be listed on your official US Club team roster printed from your secure US Club Web Site.
 
  11. ALL U15-U19 Teams  
   
- All players/teams may complete the NSR College Player Profiles.  This is a benefit of your participation in the NSR Region II Cup Tournament.  It is in additiion to your tournament roster as listed in item #7 above.  Please contact Spader at spader@nationalsoccerranking.com for more information on profile submissiion.
 
  12. Maps and Directions  
   
- Please review the maps and directions with your team families
 
  13. Over view of information Due at Registration Friday, May 6, 2011  
   
- Registration will be held on Friday, May 6, 2011 at the Radisson Hotel & Conference Center from 7:00 pm to 10:00 pm CDT.  There will be no registration at the fields on Saturday.
- Registration will be organized by your team number that is listed by your team name on your game schedule.  Please have this number available when you enter registration.
- Complete On-line Tournament Roster for administration use - must include uniform numbers (we will have these copies at registration).
- US Club Roster, USYSA (State) Roster or CSA Roster and Guest Player Form if applicable.
- NT Form for ALL US Club and CSA teams.
- Travel permission from your governing body for all out of state USYSA carded teams.
- USYSA teams with guests - provide state approved guest player form.
- Current player passes for all players and coaches.
- Copies of IYSA Medical Release forms for ALL players.  Only the IYSA Medical Release Form will be accepted.
- Emergency contact information for team manager and/or coach (hotel phone, cell phone, etc.) while attending the tournament.
- Players do not need to be present at registration.
