US Club players must be on your
official US Club roster. |

USYSA teams, if you have players guesting -
please complete the NSR guest player form. |

ALL U16-U19 Teams |

All players/teams must complete the
NSR player profiles using the passwords supplied to you by your team
coach/administrator. This is part of your tournament package. |

Registration will be held on Friday,
May 23, 2008 at the host Hotel from 6:30 pm to 10:30 pm.
There will be no registration at the fields on Saturday. |

Registration will be organized by
your team number as noted on your game schedule. Please have
this number available when you enter registration. |

Completed On-line Tournament Roster
for administration use (we will have these copies at registration). |

- |
USYSA (State) Roster or US Club
Roster and Guest Player Form if applicable. |

- |
Currrent player passes for all
players and coaches. |

- |
Copies of State Medical Release
forms or US Club Medical Release forms for ALL players. |

Emergency contact information for
team manager and/or coach (hotel phone, cell phone, etc.) while
attending the tournament. |
