(Your team Rep will show waivers at registration check-in and will

US Club players must be on your
official US Club roster. |

USYSA teams, if you have players guesting -
please complete the NSR guest player form. |

All players/teams should complete
the NSR player profiles using the passwords supplied to your team as
part of your tournament package. Contact Spader for further
information and is in addition to your tournament roster as listed

Registration will be
held on Friday, May 28, 2010 at the host Hotel from 6:30 pm to 10:00 pm.
There will be no registration at the fields on Saturday. |

Registration will be organized by
your team number as noted on your game schedule. Please have
this number available when you enter registration. |

Completed On-line Tournament Roster
for administration use (we will have these copies at registration). |

US Club Roster or USYSA
(State) Roster and Guest Player Form if applicable. |

Currrent player passes for all
players and coaches. |

Copies of State Medical Release
