Present a "copy"
of your State or US Club Medical Release Form for each Player
(depending on carded organization) ("Copies" must be left with
Tournament officials at registration. Team Representative
should keep the original copies.) |

Guest Players for
USYSA Carded Teams |

Complete a guest
player form approved by your state association. |
|
|

Guest Players for US Club Carded Teams |
|

All players should be
listed on your official US Club team roster. |

All U16-U19 Teams |

All players/teams should complete
the NSR College Player Profiles. This is part of your
tournament package and is in addition to your tournament roster as

Registration will be
held on Friday, September 3, 2010 at the Holiday Inn Select from 6:30 pm to
10:00 pm. There will be no registration at the fields on
Saturday. |

Registration will be
organized by your team number that is listed by your team name on
your game schedule. Please have this number available when you
enter registration. |

- |
Completed On-line
Tournament Roster for administration use. (We will have copies of
these rosters
at registration). |

- |
US Club Roster or USYSA (State) Roster
and Guest Player Form if applicable. |
|
- |
Travel Permission
from State Association for all out of state USYSA teams. |
|
- |
USYSA teams with guests - provide
state approved guest player form. |

Currrent player
passes for all players and coaches. |

Copies
of State Medical Release forms or US Club Medical Release forms for

Emergency contact

Payment of team parking fee:
U08-U10 is $70.00 and U11 and above is $90.00. Additional
$5.00 charge to pay by credit card. |
