Medical Release Forms |

A "copy"
of each players State Medical form or US Club form must be left with
Tournament officials at registration. Team Representative
should keep the original copies. |

Guest Player Form |

- |
US Club players must
be on your official US Club roster. |
|
- |
USYSA teams with
guest players should complete the Gold Cup guest player form. |
|

ALL U16-U19 Teams |

- |
All players/teams
must complete the NSR player profiles using the passwords supplied
to you by your team coach/administrator. This is part of your
tournament package. |

Registration will be
held on Friday, August 29, 2008 at the Hyatt Lisle Hotel from 6:30 pm to
10:30 pm. There will be no registration at the fields on
Saturday. |

Registration will be
organized by your team number as noted on your game schedule.
Please have this number available when you enter registration. |

- |
Completed On-line
Tournament Roster for administration use (we will have these copies
at registration). |

- |
USYSA (State) Roster
or US Club Roster and Guest Player Form if applicable. |

- |
Currrent player
passes for all players and coaches. |
|
- |
Copies
of State Medical Release forms or US Club Medical Release forms for
ALL players. |

- |
Emergency contact
information for team manager and/or coach (hotel phone, cell phone,
etc.) while attending the tournament. |
