the IYSA (Illinois Youth Soccer
Association) Medical Release for each player.
The IYSA Medical Release Form is available for download on the
tournament web page uner "forms". (Copies must be left at
registration - team representative should keep originals.) Copies of insurance cards are not required. |

Guest Players for USYSA Carded Teams from Outside of
Your Club |

Complete a guest
player form approved by your state association. |

All players must have a USYSA pass. |

the NSR College Player Profiles. This is a benefit of your

Registration will be held on Friday,
August 30, 2013 at the host hotel (Country Inn & Suites) from 6:30 pm to 10:00 pm.
There will be no registration at the fields on Saturday. |

your team number that is listed by your team name on your game schedule. Please have this number available when you enter registration. |
